I tried Losarten (Cozaar) this year but after being passsed on to a BP 'specialist' he pooh poohed the usefulness of it and put me on about 6 others in quick order. One day he stopped 4 at once , one of which I'd been on for years at 2 to 3x max dosage!

Cavalier comes to mind- but it's my body. To be fair- I couldn't see any improvement BP side.

No gout to assess at the time but i knew of Losartens reputation against gout already.

I'd wanted to give lostarten a good try but not to be - that time anyway.

Interesting that you find it good.

Yes ,an excretion test would be useful- but after stopping Thiazides last year by own volition, I had a year free of gout till recent. hence my appearance in force here  now ;)

PS Doc then denied thiazide connection tio gout though it is listed in the drugs' side effects :(

I guess I got complacent- and with big 'G' - that never pays, does it !

Good point about Pot BiCarb too-I will check it out…

The trouble with all this hyper drug therapy is that bad boys like sodium get elevated reputations when they are just needing 'management'- something that script drugs don't seem to warrant so much as is needed ??

Don't get me wrong, trev. By itself losartan is a DREADFUL antihypertensive, BUT for those who respond very well to thiazides (basically volume or salt sensitive hypertensives), LASIX (furosemide) is a tremendous antihypertensive. And tied to losartan the net effect is excellent for gout.

Taking losartan alone for hypertension is a mistake gout or no gout, it's just too wealk. For anyone without gout, losartan is a complete waste of money.

If you haven't read this, it's exactly to the point:

I just had a baseball weekend in Chicago with my friends which included excessive amounts of beer and meat.  I had been targeting this event as my true test to determine if Probenecid was working.  I have to conlcude that it is.  I had a great time and did what I wanted with no problems.  This comes only 57 days after having three stratight months of twinges and full-bllown attacks.

It’s heartening to see what can be done with a good gout diagnosis and proper uric acid follow-up. Smile

Way to go, coach. I hope you and probenecid have a long and fulfilling relationship!

So I've hit my first real roadblock after a vacation with my wife in New Orleans where the food and booze were enjoyed in excess, at least at first.  Upon my return home i have experienced twinges and then pain in my arch that is moving into my ankle, like my other 3 main attacks in the past year.  I'm still on Probenecid at 500 mg a day, and I've been taking 100 mg of Indocin and baking soda for the past three days.

There are two main theories that I think can explain my state of affairs.  The first is that I created many more msu crystals during my drinking which halted the excretion of uric acid.  Now I am re-dissolving crystals which is causing my discomfort due to heightened ua levels.  Or,  the hour-a-day exercise I've not stopped throughout all of this creating problems for the excretion of uric acid.

I have to admit that this development has me depressed.  I guess I was nieve enough to think that I was cured and got careless.  While this is not a full attack, the limp is back which pretty tough for a coach to hide.  “hey coach, what's wrong with your leg” is not want you want hear from your players.

Sorry apcoach, getting rid of gout is not usually plain sailing.

Remember, you probably have a buildup of crystals from before you started probenecid, so these are as likely candidates for a gout flare as your “carelessness”. You had this slight gout flare happen in May, so maybe this is just another episode on the way to getting all the old uric acid crystals out.

One thing that worries me slightly, is that you are now in your third month of treatment, but you haven’t reported a uric acid result here yet (forgive me if it’s somewhere else, and I missed it). Are you getting proper ongoing support from your doctor? Frequent tests during this stage of your gout management would make it much easier to guess the cause of a gout flare – and the chances of getting another.

Thanks again, Goutpal!  I have had my UA level checked twice since starting Probenicid.  A month in it was 5.7 and few weeks ago it was 5.4.  What do you think?  Probably old crystals re-exposed by the excess of purrines while my Probenicid was incapacitated due to the excessive alcohol, right?

I'm feeling much better than I was mid week.

I had another test and my UA was 6.3.  I've had some tingling the past week but it hasn't turned into an attack.  So my question is, is my 6.3 a result of dissolving crystals into the blood thus raising my levels?  I have to say I'm a bit discouraged by this.

Hi coach,

A 6.3 shouldn't be a cause for too much concern. Of course, given old crystallization sites and the response to acid/alkaline medium, supersaturation, day to day fluctuations, etc. who knows.

You are unique here and totally dependent on urate EXCRETION, thus you have no control over overproduction of uric acid. If you are at a break-even point with 500 mg. it might be easy to flop into unbalance with careless eating, injury, cold feet etc. I think probenecid induced excretion of urate is pretty dose-dependent and you might consider taking more…or adding allopurinol to produce less uric acid.

On probenecid dosing, you might want to consider 1,000 mg./day. Given the following, I think I would:

Doctors prescribe Probenecid after determining a patient’s allergies, drug resilience, weight and height. For the onset of gout or reoccurring gout attacks, the usual dosage for adults is 250 mg (or half of a 500 mg tablet) two times per day for seven days. After which, the patient may consume up to 500 mg two times per day for two to three weeks. Future dosage will depend on the patient’s uric acid levels in the blood and urine. The average male requires 2-4 pills per day, though some take more or less depending on uric acid levels and recovery rate. The branded form of Probenecid costs roughly $80-150 USD for 100 tablets at 500 mg, while the generic form costs $40-100 USD for 100 tablets at 500 mg.

Great News, Zip!  Your attach excerpt about dosage from the Mayo Clinic convinced my dr. to increase mine to 1000 mg a day.  We'll see how that goes.  Thank you for interest.  I love this forum!

Coaching season is now over for the year just in time for the holidays.  As previously stated, I increased my dosage to 1000mg of Probenecid on August 1.  Sometime in September I stopped having flare-ups altogether.  I am too experienced to proclaim that I am now healed, as I have done in the past.  I am, however, living a normal life with the help of this drug.  I have had no problems resuming my vigorous workout schedule and I still consume meat and beer often.  I would say that I drink less than before all of this because I want the Prob to work, but if I'm at a ballgame or party with friends I do what they do.  I'm very happy with how things have turned out thus far.  I want to thank  GoutPal again.  The guidance provided on this site influenced my doctor's decisions.  I will get my UA level taken again soon as I am confident it will be much lower than the 6.3 back in July.
