How do I know it is gout?

I am only 24 and after 8 days with a really sore foot (near my big toe) i decided to go to the doctors.  He suspected it was gout and did blood tests – i got my results last night and the Acid level was .39 (if it was over .42 then it would have been high). All other levels were fine.  I said to him what do we do now and he said wait and see if you have a attack again and we will take more blood tests.  I have still got the pain on and off in my toe and it has been 10 days now. How long can a attack last for as i am sure i have gout.  I have cut out all alcohol since i suspected gout as i have read it can bring on a attack.  I have read that blood tests won't necessarily show you have gout… I am just wondering what i should do from here.  Also, If the pain doesn't stop does that mean it isn't gout, i have read that a attack lasts from 2 – 10 days.  Its so frustrating not knowing, i was hoping something would show up in the blood test so i knew for sure that's what it was.

This can often be a tricky part of gout diagnosis.

Uric acid levels fluctuate widely, so at best you can only rely on an average of a few tests taken over time.

Regular testing in this way is very important once you know you have gout, but it is not a reliable diagnosis method.

The reason is that, during a gout attack, some of the uric acid has formed crystals and moved from your blood to your joints, or other tissues. This often results in a fall in uric acid in the blood ( serum urate).

When uric acid level is borderline, this can often lead to prolonged inflammation as uric acid crystals form then redissolve. I've tried to explain this complex process in my explanation of how uric acid causes pain.

The only certain way is arthrocentesis. This involves drawing a small amount of fluid from the joint and analyzing it under polarizing microscope. Other tests are usually done at the same time to rule out other conditions. Though this procedure sounds nasty, it's really quite painless.

I think it is vital, wherever there is any doubt about the cause of swollen joints, to get this test done as soon as possible.
