This report from Japan (2002) shows remarkably flat ( for our discussions) SUA levels- actually tailing off in the elderly, as BMI falls away.

Female BMI increases notably after 50 as gout in them become more common.

What this says to me is that actual levels of SUA are not the absolute risk and the  eternal question of co- conditions raises its head. The report mentions this issue in passing.

I would infer from first reading, that it's the co-conditions of high BMI, BP and Type 2 diabetes onset that are likely to explain increase in gout as the trend is also self limiting by mortality intervention, caused mainly by these secondary conditions.

Gouties may have worse outcomes- and not last the course , but they're not ALL dead- and SUA stays steady. In fact it falls right off after 65 for males. :)

As this is not a Gout study  per se- I agree in caution in interpretation – but there seems to be something here on the added risks of co morbid illness associated with higher SUA.
