What I am trying to understand, is if we have these crystals in our joint that build up over time,  what is it that causes a sudden attack. Do they increase in number and concentration rapidly with high levels of acid in the blood? Is there a threshold concentration that can be tolerated with out pain, and once that is breached the attack is imminent. The point is to manage gout without drugs, if that is possible for me, I want to know if I can keep acid levels down(mine are high, 9.8), and I happen to fall off the diet or drink a few beers will I be at risk. From ready thus far it seems that if I can get levels down and dissolve the crystals I will have some margin to work with.

At 9.8, 3 points above the saturation point for uric acid, you are at risk of uric acid crystals forming. Nobody knows exactly why we can achieve such high concentrations without crystals forming, but whatever the reason, there is little doubt that crystals will form eventually.

Blood flow restriction through low temperature, poor posture, or tight clothing etc., is a common trigger.

When crystals do form, the severity of the gout flare depends, not only on the number of crystals, but also on the state of your immune system and the availability of natural (and medicinal) anti-inflammatory substances.

Always bear in mind, that gout flares can occur when crystals dissolve in a low uric acid situation. See Uric Acid Crystals – Two Ways To Hurt. This is why doctors often prescribe colchicine with allopurinol – as a protective or preventative (prophylactic) measure.

So the only way to prevent harmful uric acid crystals is to maintain uric acid levels below 6.8 mg/dL permanently. This applies wether you try to control gout through medication or diet. Pain control can never be a measure of success in a gout management program – you must measure and control uric acid.
