    I am Pam, male, aged 35 years, height 164cms and weight 60 Kgs, from India. I am suffering from a variant of gout (Affecting my Foot, but not my toes) probably from the past 3-4 years, which was only detected around 12 months back. My food is vegetarian with no meat, eggs or fish.

     

    My Uric acid levels for the past one year are in the range of 9.0 to 10.0 Mg/dl. I had an attack of Gout in the last week of March (28th) and my Uric acid levels had increased exponentially to 14.0 Mg/dl when I checked on 5th April.

     

    I am having the following medication (On Doctors Advise) for past 5 days

    1. Alluporinol (Sustained Release) 250mg X 3 times daily (IE 750mg daily)

    2. Colchicines 0.5 mg X 3 times daily

    3. Indometacin only when in pain

     

    Now I want the following Info

    1. When can I expect the Uric acid levels to be back to around 6.0/dl?
    2. I smoke 5-10 cigarettes daily. Should I stop or reduce? What are the safe levels that I can have?
    3. I used to drink 200-300 ml of Rum or Whisky of 1300 ml of beer. Should I stop or reduce? What are the safe levels that I can have?
    4. My life style is sedentary IE no exercise. So does exercise help?
    5. any other advise you can give

    Wow! Mr Pam..you have got a bucket full here.   Frown What gout variant has been disgnosed?

    1.On the SUA- bear with it, 5 days is not long , Your Doc is on the case with his approach which is a nice change from usual in gout.When is your next blood test?

    2 If you value your health give up smoking. [Full stop- I'm an ex cigar man]

    3. Stop this too, at least till you get 1 & 4 right- and then moderate alcohol intake substantially, too.

    4 Increase activity daily and get a program of regular walking and swimming at least.

    5. Follow the above Cool with plenty of good quality  water intake- till it's second nature.

    Your body needs your kidneys , liver and lungs to detox – so don't rely on the meds to do the work you should be doing.

    Again WOW…that is a lot of allopurinol (but within rational bounds.)

    There is no need for sustained release allopurinol, in fact this is the first I;vew ever heard of it. For sustained release why would there be a need for 3x a day dosing.

    But to your questions:

    1. Your serum uric acid is probably below 6.0 already.

    2. You should quit smoking but it doesn;t affect gout.

    3. That's pretty heavy drinking if you do it every day. Cut back. The allopurinol is very forgiving but you have a liver to think about. Maybe 100-200 is reasonable. Beer is not a good beverage for gouties. Occasionally okay but not on a daily basis.

    4. Exercise helps in a lot of ways, but not specifically for gout.

    After about a month or two you can revisit the dosage of allopurinol with your doctor…you can probably get away with less. I would expect your current regimen will keep you free of pain. You will also propably want to get off the colchicine soon. Unlike allopurinol it is not a forever drug.

    One more time WOW!

    Alcohol big NO NO! Become friends with plain water. Exercise not the first priority I would say as you have bigger issues to solve first. Plus it's not liek you can exercise right now. You don't sound overweight….are you?

    Zip the 3 times might be to ease the stress of one big hit of AlloP on the stomach or whatever.

    you asked me What gout variant has been disgnosed?

    the Docs handwriting is a little illegible, but says my case is a simple case of EPISODIC Osteoarthritis AND SAYS that

    i should do well on simple anti inflamatory drugs.

    What's in a name? A strange way to put it- It sounds like and  is treated like gout, with SUA levels like yours.

    Maybe there is  more data on your condition -like MRI or simiilar showing independent bone damage?  …

    The good thing is you're not overweight and manage on the meds very well -whatever your Doc likes to call it!
